Referral Forms
The following online form is for veterinarians wishing to refer a patient to VCA Canada Alta Vista for overnight hospitalization, emergency and critical care services or specialist evaluation. Specialty referrals must come from a primary care veterinarian. If you think your pet could benefit from this type of advanced care please contact your family veterinarian.
Please note - A transfer form is no longer required when referring a patient to the emergency department. We ask that the referring veterinarian contact us directly to discuss the case. 613-731-9911.
