Patient Referrals  

We have recently launched a new online referral management system. Please use this to submit all required referral information including patient history, imaging and relevant lab work. For telemedicine requests, you will receive an email notification when the report is complete.

If your clinic does not already have an account, please register using the link below. After logging in with your existing account, you may add other users to your clinic under the My Clinic section or contact us for support.
